As a Behavioral Health Case Manager (CMII), you are responsible for helping to improve the functioning of clients/patients who have a wide variety of physical, cognitive, social, emotional, developmental, or age-related needs as prescribed on the treatment planYou will:
Provide all services and functions within Preferred Family Healthcare's Code of Conduct; adhere to all company policies for employment Demonstrate understanding of health service systems and assist program participants in identifying, securing, and sustaining a range of resources needed to facilitate independent healthy living.
Coordinate care and participate with the clinician in the development of the client/patient's treatment plan based on individualized strengths, needs, abilities, and preferences of the person served by conducting case management assessments.
Consult and cooperate with community resources to facilitate linkage, referral, crisis management, advocacy, and follow-up with the focus on attaining goals as identified in the treatment plan.
Provide direct services to adults, children, and families; coordinates family involvement in the service-delivery process.
Help clients/patients navigate the system of financial entitlements to ensure they have adequate income and access to healthcare coverage.
We think you'd be great for this role if you have:
Bachelor's degree in social service, education, behavioral health, or other health-related fields from an accredited college is required.
Documented experience in the area of case management services is preferred.
Staff must demonstrate current certification as a Certified Behavioral Health Case Manager (BHCM) I or II with the OK Dept.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
